This form is for external doctors to use if they request the Diabetes Educator start a patient on insulin.

Bus routes 216, 219 and 220 all stop at the corner of Commercial and Punt Roads.
Tram number 72 stops at the corner of Commercial and Punt Roads.
Tram numbers 3, 5, 6, 16, 64 and 67 travel along St Kilda Road. Alight at the corner of St Kilda Road and Commercial Road, and walk approximately 600 meters on Commercial Road to reach the clinic.
Train on the Sandringham line, alight at Prahran station and walk north along Charles Street to Commercial Rd. Turn left onto Commercial Road and continue toward Punt Rd (approximate walking distance 900 metres). Pakenham line, alight at Toorak station and catch Tram Number 72 to the corner of Commercial Road and Punt Road.
Underground parking is available on-site, however availability can be limited at peak times (fees apply).
Two hour street parking is available in some nearby streets.
Casual parking is available at Prahran Market, a 15 minute walk along Commercial Road (fees apply).
The specialist clinic operated by the Baker Institute at 5 Stanlake Street, Footscray, will be moving to the cohealth facility at 78 Paisley Street, Footscray, from 1 September 2017. This means that from this date, patient appointments with our diabetes specialists will be at this new address.

The Footscray team includes four specialist diabetes physicians, a diabetes nurse and health education professionals. The team is led by Associate Professor Neale Cohen.
The clinic is open Monday to Friday, 9am to 5pm.
