Based on the ability of echocardiography to assess haemodynamics and myocardial function, we are seeking to contribute to the reduction of heart failure (HF) admissions and costs by:

  1. Developing a risk algorithm for HF readmission, including psychosocial determinants.
  2. Combining the knowledge derived from data linkage and the risk algorithm to devise a new means of providing a HF disease-management program (DMP), calibrated to risk assessment, and performing a clinical trial to define the merits of this approach.
  3. An evaluation of cost-effectiveness of this intervention.
